Unsuccessful setting SL and TP

Hello.

I am using my strategies on MT4 and all of them have ATR based SL and TP and open trades at market. Sometimes I see that some of the trades of the same EA has not set SL and TP, I checked the MT4 journal and I found that the EA has tried to modify the order successfully without any errors. I mean after opening the order, it modified it just a moment later but were not set on the trade, then I set them manually with the prices which I see on journal for SL and TP.

What can might be the problem?
